through Godwin Mortgage Group, Inc.'s website, you
acknowledge receipt of this document and consent to
the electronic delivery your information.
You authorize Godwin Mortgage Group, Inc. to order
a consumer credit report. The information Godwin
Mortgage Group, Inc. obtains is only to be used to
pre-qualify borrowers for financing and should not be
considered as a commitment or approval of such
financing. You authorize Godwin Mortgage Group,
Inc. and its affiliated business partners to contact
you by phone, email and regular mail regarding your
Request.
Request from any source named in this Request,
and Lender, its successors or assigns may retain the
original and/or an electronic record of this successors

secured by a mortgage or deed of trust on the

audio and video recordings), or my facsimile
transmission of this application containing a facsimile
of my signature, shall be as effective, enforceable
and valid as if a paper version of this Request were
delivered containing my original written signature.
